Copy of Report Cards (full record from last completed school year & any progress reports/report cards from the current school year)
Recommendation Form to be completed by current teacher (select the appropriate form)

Each application must be accompanied by a $50 Testing/Screening Fee (checks payable to St. Mary of the Hills School). This fee is non-refundable. After school has closed for the summer, the Testing/Screening fee is $75.
Once all required forms have been submitted, the applicant will be given a screening or testing date.
